The annual Form 1095C form documenting Employer-Provided health insurance offer and coverage is now available electronically beginning Friday, January 30, 2026. Employees may view and access forms directly in the UCPath portal. Individual email notifications have also been sent notifying the forms are available for viewing and printing.
Instructions for Current Employees:
Current employees may access the 1095-C form by logging into the UCPath portal and click on the View Online 1095-C link in the Income & Taxes dashboard.
Instructions for Former Employees:
Former employees may access and download the 1095-C form for up to three years following separation from employment. To access the form, visit the UCPath website and click on the Former Employees link. After logging in, navigate to Income & Taxes and select the corresponding tile for the desired form.
Please keep this timeline in mind should you receive inquiries from employees or former employees.
Important security reminder: The University of California does not send W-2 statements by email or text. If an individual receives an email or text with a link or an attachment to view a W-2 statement, it is a phishing scam designed to gain private information. Please advise employees not to open attachments or click email links claiming to provide access to W-2 statements.
